Application for Issuing an SFC IC Card ( for Delta Building / Nu Building / Zeta Building )

Faculty members and researchers who wish to obtain an SFC IC Card should download the application form below and submit it to the Property Administration Section of the General Affairs Office after filling in the necessary information (the form for students is available in Japanese). Administrative staff will contact the applicant based on the contact information provided in the application form when the card is ready to be picked up.
* Applicants must obtain a seal of the person in charge of the party (research group's representative) on the application form.
