Campus Card

Faculty members and researchers who wish to obtain an SFC Campus Card should download the application form below and submit it to the Property Administration Section of the General Affairs Office after filling in the necessary information. Administrative staff will contact the applicant based on the contact information provided in the application form when the card is ready to be picked up.
* Applicants must obtain a seal of the person in charge of the party (research group's representative) on the application form.
